You did connect the ignition wire? (no disrespect- it has happened!). Otherwise go through the diagram posted above (by Kiwi » Fri May 24, 2013 1:43 am). When you find the right combination please post it here for the general knowledge as someone else may have the same issue at some time. Cheers.

He programmed mine as I asked, but I had to change some settings straight away, but it's easy. It may take a couple of goes to get the button press timing right, but its easy enough- you just need the cable.

On my last controller I'd used a plastic constructors box with rubber grommets to house all of the connectors that congregate a few inches from the controller. What do you guys do? I was considering a neoprene sleeve with cable ties, but any other options?

I use the neat easy to use Hobbyking AS150 anti-spark connectors , resistance is lower (around 5 ohms from memory) but they work fine- I've tried up to 72V (80V fully charged). (1 set is $8.49) They now have XT90-S Anti-Spark Connectors too - I'd probably have chosen these if they had been available...

I went through the same to get the correct combination for my motor. What motor are you running? Someone may have the correct wiring for you. As a workaround for now, try to enable the SoftLVC (uncheck "AfterLVC Off") and set the AfterLVC Amps to max (20A). we should have a solution soon on this.
